Registration Information (Please Read Carefully)

1. Registration requires a completed team roster (see reverse side) and paid fees on or before the deadline, before participation in the league.  (No exceptions)

2. You must have the minimum of 5 players on your roster but no more than 10.

3. Note: you must have the minimum number of players per team to take part.  Players in a WIAA sport during that season are unable to participate in leagues.

4. All participants must provide their current grade, age and valid school ID Number. Must be in high school to participate. 9th grade to 12th grade or Staff member.

5. All team managers or a member of your team must attend the mandatory manager’s meeting before participation in the league.  (No exceptions)

6. Before all scheduled games players must show proof of school I.D. to compete.

7. Home Schooled students or other high school students must show proof of I.D. or note from parents that they can participate.

Liability Waiver/Release

As a member of the above stated team, I hereby attest that I am participating of my own free will in this league/activity sponsored by the Menomonie High School , and exclude the sponsored agency and it’s paid and volunteer staff from liability in the event of an accident or injury resulting from participation under normal circumstances.
